Pulmonary tuberculosis (TB) remains a significant public health problem in Indonesia, with high morbidity and mortality rates. One of the most common complications experienced by TB patients in the ICU is ineffective airway clearance caused by excessive and thick secretions. This case study aims to describe the nursing care process provided to Mr. N, who was diagnosed with pulmonary TB and presented with ineffective airway clearance. Conducted from September 1–3, 2025, this study utilized a case study approach encompassing assessment, nursing diagnosis, interventions, implementation, and evaluation. The primary intervention was regular suctioning through an endotracheal tube, supported by oxygen therapy, semi-Fowler positioning, and collaborative pharmacological treatment. The results indicated progressive improvement over the three-day period, demonstrated by reduced sputum production, enhanced ventilation, improved respiratory patterns, and increased oxygen saturation up to 100%. These findings confirm that suctioning is effective in managing ineffective airway clearance among critically ill TB patients and can serve as an evidence-based practice in respiratory nursing care in intensive care settings.
