Abstract

Background: Hypertension is one of the most common non-communicable diseases among older adults and remains a major public health concern because it can lead to cardiovascular complications, stroke, kidney disease, and reduced quality of life. Effective hypertension prevention requires not only individual behavioral changes but also active family involvement as the primary source of support for older adults. Family-based education is considered an effective promotive and preventive strategy to enhance knowledge, attitudes, family support, and hypertension prevention practices. Objective: This study aimed to determine the effect of family-based education on hypertension prevention among older adults in the working area of Lipat Kain Primary Health Center, Kampar Regency. Methods: This study employed a pre-experimental design using a one-group pretest–posttest approach. A total of 38 respondents, consisting of older adults diagnosed with hypertension and their family caregivers, were recruited through purposive sampling. The intervention consisted of family-based educational sessions supported by educational modules and leaflets. Data were collected using validated questionnaires measuring knowledge, attitudes, hypertension prevention practices, and family support before and after the intervention. Data were analyzed using the paired t-test with a significance level of 0.05. Results: Family-based education significantly improved respondents' knowledge, attitudes, hypertension prevention practices, and family support (p < 0.001). The mean scores of all measured variables increased significantly after the intervention, and all respondents demonstrated improvement toward good hypertension prevention behavior following the educational program. Conclusion: Family-based education has a significant positive effect on improving hypertension prevention among older adults through enhanced knowledge, attitudes, preventive practices, and family support. Integrating family-based educational interventions into primary healthcare services is recommended to strengthen community-based hypertension prevention programs for older adults.
