Abstract

Indonesia's 2021 health profile states that the national coverage of complete basic immunization is 84.2%. The target of the 2021 strategic plan is 93.6%, so this figure has not met the target.  Community empowerment is a social, cultural, psychological and political process through individuals and social groups so that they express needs, present concerns, strategize participation in decision making, and take political, social and cultural actions to meet health needs (WHO, 1998 cit. Sulaeman, 2013). The target in community empowerment that will be carried out is mothers who have infants and toddlers. The precede proceed model is one of the models used to plan and evaluate health promotion. The stages in the implementation of this model can be carried out sequentially, are flexible and can adapt to needs. The Precede phase consists of five phases, including a comprehensive assessment process consisting of social assessment, epidemiological assessment, behavioral and environmental assessment, educational and ecological assessment, administrative policy assessment. The results of the assessment of the five precede phases obtained priority health problems, namely the low knowledge of mothers about immunization. The Proceed phase has conducted counseling on immunization to mothers who have babies and toddlers in Lubuk Siam village. The results of the counseling conducted found that there was an increase in maternal knowledge before and after the counseling was conducted.
