Lower back pain is one of the most common complaints experienced by third-trimester pregnant women, with a global prevalence of 50–80% and reaching 60–80% in Indonesia. A preliminary study conducted at BPM Affah Fahmi Surabaya revealed that out of 40 pregnant women, 32 (80%) experienced lower back pain. Non-pharmacological interventions such as pelvic tilt exercises and the application of rose essential oil are considered safe and effective alternatives for pain reduction. This study aimed to examine the effectiveness of combining pelvic tilt techniques and rose oil in reducing lower back pain among third-trimester pregnant women. The research employed a quasi-experimental design with a two-group pre-test post-test non-equivalent control group approach, involving 30 participants divided into two groups: intervention and control. The intervention group 15 received pelvic tilt exercises three times per week (3–5 minutes per session) and topical rose oil application twice daily for one month. The control group 15 received educational counseling only. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention. The results showed a significant reduction in pain intensity in the intervention group compared to the control group (p<0.05). In conclusion, the combination of pelvic tilt exercises and rose oil is effective, safe, and practical as a complementary midwifery intervention. It is recommended that healthcare providers educate pregnant women to independently apply this intervention to alleviate lower back pain during the third trimester.
