Perineal laceration represents a frequent complication in vaginal births, potentially leading to bleeding and impaired reproductive organ function. Approximately 85% of women undergoing vaginal delivery are at risk of experiencing perineal laceration. A non-invasive preventive measure includes perineal massage. This research seeks to evaluate the differences in the severity of perineal laceration between delivering mothers who receive perineal massage and those who do not. The study employed a quasi-experimental design utilizing a posttest-only control group approach. The sample comprised 26 delivering mothers, split into an intervention group (13 woman receiving perineal massage) and a control group (13 woman without intervention). Data were collected at PMB Rosita and Ernita Midwife Clinic in Pekanbaru City from April 24 to May 23, 2025. Data analysis was conducted using the Mann-Whitney test. The results showed that in the intervention group, the majority had no laceration (61.5%), followed by grade I (30.8%), and grade II (7.7%). In the control group, the predominant outcome was grade II (61.5%), grade I (30.8%), and no laceration (7.7%). The Mann-Whitney test yielded a p-value of 0.001, confirming a statistically significant difference between the groups regarding perineal laceration severity. Conclusion: As a non-pharmacological alternative, perineal massage demonstrates significant efficacy in lowering the degree of perineal laceration among delivering mothers. It is recommended that perineal massage be incorporated as a key intervention in maternal healthcare services to mitigate the risk of perineal rupture during childbirth.
