Tonsillectomy under general anesthesia is a surgical procedure that may lead to postoperative anxiety due to pain, limited activity, and uncertainty regarding patient outcomes. Anxiety that is not adequately managed can delay recovery, increase pain perception, and add to the burden of care. Therefore, safe, practical, and effective non-pharmacological interventions are needed to address postoperative anxiety. This study aimed to analyze the effect of instrumental music therapy as a non-pharmacological intervention on reducing anxiety levels in post-tonsillectomy patients. This study employed a quasi-experimental design with a one-group pretest–posttest approach. The study was conducted at Wijayakusuma Hospital Purwokerto from June to July 2025, involving 30 post-tonsillectomy patients aged ≥17 years who met the inclusion criteria of having no complications, being cooperative, and willing to participate. Anxiety levels were measured using the State Anxiety Inventory (STAI-S) before and after a 15-minute instrumental music therapy intervention. Data were analyzed using the Wilcoxon Signed-Rank Test. The results showed that before the intervention, most respondents experienced moderate anxiety (96.7%) and severe anxiety (3.3%), whereas after the intervention, the majority experienced mild anxiety (96.7%) and moderate anxiety (3.3%). The Wilcoxon test revealed a p-value of 0.000 (p < 0.05), indicating a statistically significant difference in anxiety levels before and after the intervention. In conclusion, instrumental music therapy is effective in reducing anxiety among post-tonsillectomy patients and is recommended as a supportive non-pharmacological intervention in perioperative nursing practice.
