PENERAPAN HEAD UP POSITION 30° PADA PASIEN STROKE NON HEMORAGIK DENGAN PENURUNAN KAPASITAS ADAPTIF INTRAKRANIAL: STUDI KASUS

Non-hemorrhagic stroke is a major cause of morbidity and mortality, primarily due to impaired cerebral perfusion and increased intracranial pressure (ICP). Elevated ICP reduces intracranial adaptive capacity, leading to decreased consciousness and neurological dysfunction. One of the effective and easily applied nursing interventions to reduce ICP is the 30-degree head-up position. This case study aims to describe the application of the 30° head-up position in Mrs. M, diagnosed with non-hemorrhagic stroke and experiencing decreased intracranial adaptive capacity. The study was conducted in the ICU of Kardinah General Hospital, Tegal, from September 1 to 3, 2025, using a case study approach covering assessment, diagnosis, intervention, implementation, and evaluation. Data were collected through observation, interviews, physical examination, and documentation review. The head-up 30° intervention was administered daily with continuous monitoring of vital signs and consciousness levels. The results showed gradual improvement, indicated by reduced restlessness, an increase in the GCS score from E2M2V2 to E4M5V2, and a decrease in blood pressure from 194/87 mmHg to 147/88 mmHg. In conclusion, the 30° head-up position is effective as a non-pharmacological intervention in reducing intracranial pressure and improving cerebral perfusion in patients with non-hemorrhagic stroke.
