Prof. Hospital Data Dr. Tabarani Pekanbaru medical records have not been functioning properly in accordance with the MOH on guidelines and procedures organizing medical records of hospitals in Indonesia, because the system is still working concurrently in the processing of medical records at the hospital Prof. Dr. Tabrani Pekanbaru, as for the management of medical records which consisted of one patient acceptance of the queue at the time of registering patients during the day, while the processing of medical records consist of assembling, coding, filling systems, indexes, statistics and reporting hospitals, medical record analysis. This study aims to determine the Factors Affecting Medical Record Function in the Hospital Prof. Dr. Tabarani Pekanbaru. This study used a qualitative research, the research subjects Room Head Unit Medical Record, Medical Record Staff, Personnel / HR Prof. Hospital Dr. Tabarani Pekanbaru.Pengolahan data using triangulation of data, and then analyzed using qualitative analysis. The results of the study that the function of medical records at the hospital Prof. Dr. Pekanbaru Tabarani not implemented optimally this is caused by several factors namely Human Resources officers are only 1 medical record DIII-educated medical records, medical records SPO has not run properly, Infrastructures that are sufficient, processing medical records which has not been done according to the standard MOH RI even still there were not implemented at all, which is why the function is not optimal medical records, medical records clerk job description is still not in line with what is currently implemented, because all the work is done by the data processing only 1 the medical record DIII educated medical records.
