At the primary healthcare level, acute respiratory infections (ARI) remain a common public health problem, particularly in areas with high environmental risk. The purpose of this study was to determine the effectiveness of the ARI Control Program at the Tirto II Community Health Center in Pekalongan Regency. The study was designed with an evaluative design and descriptive qualitative methods were used. Four officers were responsible for the ARI program: a doctor, a program manager, a health promotion officer, and the head of the community health center. Informants were randomly selected. Data were collected through interviews and secondary document reviews, including program activity reports, 660 ARI cases in 2024, and achievement of ARI Minimum Service Standards (SPM). Qualitative and quantitative data analysis was conducted thematically and univariately. The results showed that the ARI control program was not running well. Approximately 40% of targets faced major obstacles in the input aspect, which included limited human resources and educational media; the process aspect, including irregular counseling activities, uneven early detection, and inconsistent program documentation; and the output aspect, including lack of new health education coverage. The higher number of ARI cases among boys and toddlers demonstrates the importance of strengthening community-based preventive and promotive interventions. To improve the efficiency of the ARI control program at the Tirto II Community Health Center, capacity building of cadres and health workers, provision of adequate educational resources, and cross-sectoral coordination are needed to support the achievement of the ARI Minimum Service Standards (SPM) targets
